In order to utilize your health insurance for Physical Therapy, a referral from physician is required. If you think you are in need of treatment, the first step is to consult with your doctor. Upon consultation you can specify you'd like your doctor to refer you to StayFIT Physical Therapy. If your doctor faxes the referral directly to us, we will contact you that same day or the following to schedule an appointment for you. If the doctor gives you the physical copy of the referral, please call ahead to let us know you have it and we will schedule a time for you to come in.
